Pay $16.00 - 17.50 hour

Core Responsibilities 

• Completing initial waiver service intakes with people we support and their families, identifying individual habilitation needs and family support needs.
• Develop and monitor individual Staff Action Plans for people receiving services through the OPWDD HCBS Waiver program and revised as needed.
• Promote independence in the people we support by developing person centered programming.
• Respect the privacy and home environment of the people we support.
• Coordinate facilitation of group activities within the waiver department.
• Delivery of habilitation services to individuals while assessing appropriateness of plan as developed, progression/regression, etc. Attend to the person’s needs and implement services as outlined in their Staff Action Plan. A minimum of 18 hours per week of direct service delivery will be required to be provided.
• Complete weekly/monthly quality assurance review of staff’s documentation and follow up as needed to improve quality of documentation, service delivery, and all other issues/concerns that may arise.
• Complete annual and semi-annual reviews of the people we support service plans and participate as a member of their planning team in the person’s Life Plan meetings.
• Supervision of direct support staff; including performance, initial and on-going training; and coordination of staffing approved hours.
• Maintain active charts for the people we support; ensuring all documentation is current and meets regulatory requirements; and that approved units are provided to the person.
• Participate in leadership activities as part of the Community Services team; including meetings, trainings, and committee involvement.
• Ensure positive and effective communication within and between department, division, agency, family and community personnel.
• Establish a work environment that creates positive communication between supervisors and employees; and assists new employees with assimilating to the Agency’s culture, values, and mission.
• Ensure Compliance with all pertinent government and agency requirements.

Qualifications 

• Associate’s Degree in human services or related field (I.e. psychology, sociology, etc.)
• Minimum of 12 months experience in working with people with developmental disabilities.
• Ability to lift and/or assist people in your care and the ability to meet physical requirements of the job.
• Basic Computer skills (Windows, OneDrive, Microsoft word, excel, adobe, internet and email) and the ability to learn agency software systems.
• This position has regular and substantial unsupervised and unrestricted physical contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under OPWDD (Office for People with Development Disabilities) or OMH (Office of Mental Health).
• Must have valid NYS driver's license and reliable transportation
